global_platform: fix help description for establish_scp03
The argument parser object for establish_scp03 (est_scp03_parser) is
copied from est_scp02_parser. This object still has the .description
property set, which is the description for establish_scp02. To get
the description string that is defined in do_establish_scp03, we must
remove the old description string first.

ara_m: use class byte of current lchan
The ara_m commands use APDUs with a fix class byte (0x80). This means
that all ARA-M related features only work in the basic logical channel.
To fix this, let's compute the class byte for the current logical channel
dynamically inside the send_apdu methods of SimCardCommands. This will
fix the problem globally.

I have now reviewed the code locations where we call send_apdu* methods. I have corrected TERMINAL PROFILE, ENVELOPE, and SUSPEND. As far as I can see FETCH and TERMINAL RESPONSE are not affected, since they work one layer below.

I didn't forget about this, the new patch set versions are probably created because I have re-based the patch set.
Thanks for pointing me at the table. This is really confusing: In section 11.2.1 (TERMINAL PROFILE) they say CLA As specified in clause 10.1.1. And then in 10.1.1 they happily describe the coding of logical channels. Table 10.5 is in section 10.1.2, which is also referenced, but for the INS byte. In any case, now I know where to look at.
