I'm not entirely convinced. […]
my idea was testing a reader that may (falsely) use INS = GET_RESPONSE to decide to receive two status words or to receive INS + DATA + 2xSW. i tried it with omneikey  and ccid-firmware, and it is handled correctly. it is ok with me to abandon the patch.
