There is already code doing that, using the BT-SAP interface:

it's written in ruby, but it was just for demos purposes and works.
- you can provide a SIM from the PCSC interface
- you can read a SIM using the BT-SAP interface
- you can simulate a SIM (softSIM)
- ...
