library/DIAMETER_Templates: rework existing ULR/ULA templates

* Make both {hbh,ete}_id parameters last among the others;
* Use more elegant and consistent template formatting;
* Make Origin{Host,Realm} values configurable;
* Make DestinationRealm value configurable;
* Make SessionId value configurable.
